Fertility preservation can be a great option for transgender parents

If you haven’t begun to transition yet or you’re just getting started on your journey, you may want to explore fertility preservation. Sometimes, hormone replacement therapy (HRT) can make it more difficult to conceive later. As a result, it can be helpful to freeze eggs, sperm or embryos for future use.

Egg freezing involves just a few simple steps. Patients with ovaries will take injectable fertility medications to encourage the body to develop multiple eggs at the same time. When the eggs are mature, one of our doctors will retrieve them during a short outpatient procedure. Embryologists in the lab will then flash-freeze these eggs and store them for the patient to use later.

Sperm freezing requires a patient with testes to provide a semen sample, typically through masturbation. Once our andrologists have the semen sample, they will assess its quality before freezing it to store for the patient to use for future pregnancy attempts.

Embryo freezing is a great option for couples who know they want to have children together as well as for people who want to use an egg donor or a sperm donor. This fertility option follows the steps involved with egg and sperm freezing. However, instead of freezing the eggs and sperm, embryologists use the sperm to fertilize the eggs. They then freeze the resulting embryos.

Patients who did not pursue fertility preservation before transitioning still have plenty of options to become transgender parents. Some people are even able to conceive naturally after temporarily pausing their hormone replacement therapy.

If ovarian or testicular function does not return after pausing these medications, our LGBT fertility clinic team will likely recommend one of the following transgender fertility treatments.

  • Fertility medications like Clomid may be able to help a patient ovulate, making conception possible with timed intercourse.
  • Intrauterine insemination (IUI) is a type of artificial insemination that patients can use with or without donor sperm. Some patients will have a cycle with fertility medications, while others can have a so-called natural cycle.
  • In vitro fertilization (IVF) is a more advanced fertility treatment. Patients can use their own reproductive materials during an IVF cycle or rely on donor eggs or donor sperm. It’s also possible to conceive with help from a gestational surrogate and IVF.
